Job Description

• Care Coordination: Collaborate with physicians, nurses, and departments to ensure integrated and effective patient care.

• Case Monitoring & Evaluation: Follow up on clinical cases, especially high-priority patients, and analyze data for quality improvement.

• Care Plan Management: Ensure accurate documentation and proper implementation of care plans.

• Discharge Planning & Patient Flow: Facilitate early discharge and optimize hospital stay duration.

• Reporting: Generate daily and monthly reports on key indicators (occupancy, length of stay, utilization, etc.) with actionable insights.

Requirements

• Bachelor’s degree in Medicine or Pharmacy (Mandatory)

• 0–2 years of experience in Case Management or a related field (Entry Level)

• Awareness of Clinical Practice Guidelines (CPGs)

• Preferred: Diploma or Master’s in Hospital Management
