Job Description
- Coordinate and manage patient care plans, ensuring timely and effective delivery of healthcare services.
- Serve as the primary point of contact between patients, healthcare providers, and pharmacy staff.
- Facilitate communication among multidisciplinary teams to optimize patient outcomes.
- Monitor patient progress and follow up to address any concerns or changes in care needs.
- Assist patients in navigating healthcare systems, including scheduling appointments and arranging referrals.
- Maintain accurate and up-to-date patient records in compliance with regulatory standards.
- Educate patients and their families about medication regimens, treatment plans, and available resources.
- Identify and resolve barriers to care, advocating for patient needs and preferences.
- Collaborate with insurance providers to verify coverage and obtain necessary authorizations.
- Participate in quality improvement initiatives to enhance patient satisfaction and operational efficiency.
Requirements
- Bachelor’s degree in healthcare administration, nursing, pharmacy, or a related field.
- Minimum of 3 years of experience in care coordination, patient advocacy, or a similar healthcare role.
- Demonstrated ability to manage multiple cases and prioritize tasks effectively.
- Excellent communication and interpersonal skills for interacting with patients and healthcare professionals.
- Strong organizational skills with attention to detail and accuracy.
- Proficiency in using electronic health records (EHR) and healthcare management systems.
- Ability to work independently and collaboratively within a fast-paced environment.
- Knowledge of healthcare regulations, privacy laws, and best practices.
- Problem-solving skills to address patient needs and resolve issues efficiently.
- Fluency in English; additional language skills are a plus.
