Job Description

  • Coordinate and manage patient care plans, ensuring timely and effective delivery of healthcare services.
  • Serve as the primary point of contact between patients, healthcare providers, and pharmacy staff.
  • Facilitate communication among multidisciplinary teams to optimize patient outcomes.
  • Monitor patient progress and follow up to address any concerns or changes in care needs.
  • Assist patients in navigating healthcare systems, including scheduling appointments and arranging referrals.
  • Maintain accurate and up-to-date patient records in compliance with regulatory standards.
  • Educate patients and their families about medication regimens, treatment plans, and available resources.
  • Identify and resolve barriers to care, advocating for patient needs and preferences.
  • Collaborate with insurance providers to verify coverage and obtain necessary authorizations.
  • Participate in quality improvement initiatives to enhance patient satisfaction and operational efficiency.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, pharmacy, or a related field.
  • Minimum of 3 years of experience in care coordination, patient advocacy, or a similar healthcare role.
  • Demonstrated ability to manage multiple cases and prioritize tasks effectively.
  • Excellent communication and interpersonal skills for interacting with patients and healthcare professionals.
  • Strong organizational skills with attention to detail and accuracy.
  • Proficiency in using electronic health records (EHR) and healthcare management systems.
  • Ability to work independently and collaboratively within a fast-paced environment.
  • Knowledge of healthcare regulations, privacy laws, and best practices.
  • Problem-solving skills to address patient needs and resolve issues efficiently.
  • Fluency in English; additional language skills are a plus.
